K&T 2H reverse problem

Hello all,
I was recently doing some horizontal milling with two 6" cutters taking a medium cut in mild steel. I had set up the job on the left side and needed to run the cutters with the spindle in reverse. I was running at 2.75" p/m and all was well, I decided to up the feed rate by one step and it was cutting fine until it jumped out of reverse which stopped the spindle.
I dropped it down in feed rate and pulled the handle to get it back into reverse, which worked but it did jump out two more times and then seemed to settle down and continue cutting. I was only doing six pieces so not a very long run of parts.

I have never had it jump out in the forward or "normal" spindle rotation.

Any ideas on what happened or if there is a problem with the gear train?
