what ratio do you need ?
for small / normal ratios, you may find such products as stock at some vendors
for high ratios, than it depends :
... if tool diameter is little, thus cutting efort is low, than simply consider something driven by coolant
... if cutting efort is high, thus you need to maintain a greater torque, you need a custom solution :
...... you may take a normal live holder and modify it, thus adding another spindle on top
...... you may consider a full custom holder
so, what cutting specs do you wish to target ? with what tool ?
and please, what is the default live holder collet size ? kindly

I haven't seen any speeder units that are designed explicitly for live tool use, but most live tool manufacturers have models with different drive ratios to gear up or down, 1:2, 1:3 etc.
