NL2000 gets coolant in hydraulic oil

I'm operating a NL2000 with a Kitagawa actuator. We have had the machine "puke" either from the actuator, or from the hydraulic tank what I can only describe as "pancake batter". With further inspection, it was determined that there was coolant in the oil. Where on this machine can low pressure coolant get into the high pressure oil system?:scratchchin:
 
Too much coolant collecting in the back end of actuator can allow some coolant to infiltrate the return (unpressurized) side of the actuator going back to the hydraulic reservoir. If the coolant drain going back to the tank from the actuator is plugged it can cause this problem.
 
I've seen many machines where the way covers were worn out and coolant would leak on top of the hydraulic reservoir.
