See if you can post some photos of the PCB.

Odds are quite high that it was made FOR W.W. Grainger AKA Dayton by K-B-Penta or one of the other major drive makers, and that someone can ID which, and which family by that, plus the label or power specifications.

Usually, you'll be able only to find a manual for connecting and configuring it. Not one that supports board-level repair. Not worth the effort.
