All of our cutters came from Illinios/Eclipse. They've been out of business for years. I'm not aware of any US makers.

Wouldn't the worm be cut in a lathe with either Diametral or Modular pitch change gears?

Gleason makes worm and worm gear hobs.
The WORM GEAR hob is typically a steel (8620) dupicate of the worm except it's teeth are gashed to present a cutting edge to cut the teeth of the bronze worm gear.
WORMS for worm and worm gear sets tend to be ground on a thread grinder. Worm GEARS tend to be hobbed. The worm is just a modfied screw thread, with the possibility of having mutiple starts for different ratios.
I get several gear trade magazines and there are certainly more than ONE company manufacturing gear hobs for worm gears.

You might try Ash Gear. I don't know how or if they deal with individual sales but they seem to have just about everything gear tooling related.
