Maybe it was Monday morning and the operator was still "bombed" from the weekend. They say (whoever 'they' are) that most industrial accidents occur on Fridays and Mondays. If that sucker had been fused it certainly would have disturbed your television viewing especially since there was likely a goodly number just like that one inside the building.
