Another Motor wiring needed please

hello , I started to rewire my 9A today & about halfway through the phone rang & that was the end of my progress . I was going 1 wire at a time ,remove & replace .
I have the Furnas D2058 forward reverse switch, & a Standard Electric 1/2 hp motor Mdl 73 form the Standard Dayton Corp.
Here is as far as I got before I got derailed . 6 poll switch , 4 wire motor without the ground . I will run a seperate ground due to having a piece of 14/4 from the switch to the motor.
looking at the switch, lever in front
Right Rear,T4 from motor
Center Right, jumped to Right Rear T4
Front Right #8 from motor
Left Rear ,Line in Hot.
so that leaves #5 & T1 & Left center & left Front . I think that T1 was at Left front ,so I don't know about #5 & the line neutral . & I think there was a jumper on the left side also .
Any help greatly appreciated

I got it , opened up the old lathe & spent some time , (well a bunch ) of time with some meters & it all came together other than none of my Fluke's like 19 deg , had to keep them near the heater so the lcd screens would work
