I don't recall the dimensions of the different M-head tapers (there were three, at least) but you can Google them. Start with B&S 7 and "B3".

(My old BP has a 2MT quill, and the endmill moves in the collet before the quill itself if I try to go too heavy. So, I wasn't aware of the problem.)

The quill lock, as I understand, functions somewhat like a brake. As the mating surfaces wear, the angle of the lever will have to be increased in order to give enough pressure to the lock. The threads might be the most susceptible...
Your gap will depend upon how much lash needs to be removed. I did this a few years ago, and I was foggy on it then; I hope one of the BP zen masters will respond. I would like to understand the process better m'self.
