Adjustable car stem headrest?

I have a friend with fused cervical vertebra. He is frustrated with the headrests in his vehicle because they don't enable him to actually put his head against them while driving. Im curious if there is a type of hinge that could be incorporated into each step to allow forward/backward adjustment?

Volvo had different thickness cushions that pushed into the holes in their headrest. Ask your dentist.
Be very careful anything like this has to be strong but not snap or bend under shock loading. Any adjustment must lock solid each time and click solidly into place at each notch. If it feels like it locked in but fails under load whose fault is that? I think a twist knob is out of the question since the user can not verify torque each time it is moved.
I put Volvo seats into two cars along with the haderests. They had a lot of adjustable features for comfort. the exiting seats were shot and needed rebuilding or replacing anyway. Did I really bolt them solidly enough to the floor? Who knows. I used angle iron from the front to back bolts under the floor pan to spread the load.

Some mustang headrest are adjustable. some headrests can be reversed, but that usually moves it farther back. An upholstery shop can add padding and a new cover.

Yes, I'd want something with some sort of positive lock indexing notches for certain.

Most headrests suck....but bear in mind they are not actually meant to be something to rest your head on...they are there to prevent your head from folding over backwards if you get rear ended.

Some steel rod and a welder and a vise...you can make them do whatever you like.

Thinking some more it would not surprise me if the main stem is designed to bend under extreme load rather then stay straight and transfer full load force into your skull. Kind of a crumple zone for you necks protection. Use once and throw away.
