It it hard to find BT holders that aren't AD+B, where B is through flange coolant, and using face contact holders for everything is too expensive. Some BIG+ spindles have small holes for airflow on the face to keep it clean. However, if a grub screw blocking the through-flange hole fails, coolant will be shot straight inside the spindle through these holes. Has anyone ever had this happen?

I'm not sure what you are asking? But there are relatively affordable dual contact holders available. What style are you looking for? What country are you based in?

I've recently been testing some Bilz shrink fit (with coolant bypass jets) that come in around £120 each?

I wouldn't think the face purge air uses the same path as for the spindle bearings so no course of ingress with the coolant there, at least I hope not. I have had coolant get past the pull stud and purge the grease out of the spindle bearings.
