Machine wiring Q's

Working through some new controls on a '43 Pacemaker lathe and have some q's-

1) Was it common back in the day to only switch 2 of the 3 legs going to a motor?

2) I want to add circuit breaker to a 3p 12ga circuit fed by 40a service, there isn't one currently. Which trip curve should I choose? It's for the coolant pump.

3) What the heck is with this fuse box for the lighting circuit? It looks like to phases are bonded together? See pic.

I don't know how common it was, but I have seen 3 phase motors where only 2 legs are switched, and 3rd leg stayed hot to motor, don't think anybody does that today. Not sure what is happening in that fuse box.
