It came boxed up with a South Bend lathe but I suspect that’s not it’s origin.

It is not difficult to enlarge a picture like that if it has good enough resolution. I did and can say it has some similarity to a Heinrich cross hole drilling jig, not in appearance, but in possible function.
Note the lever that probably clamps the work and the V-block. A top view might reveal a place for a drill bushing.
