    plus pinch points, gears, belts, people's removing guards to ease operations, thinking they are tough enough to not need safety glasses or a duct mask, looking away from the work, having a radio so not hearing the job change sound, talking to someone when they should be paying attention.

    You sure learn well with running a SG or Blanchard with a chuck full of small parts...you watch sparks and listen for the change of sound.
