Charmilles Robofil 440 Re-thread Issue

Got an issue with a Robofil 440, the machine will cut and re-thread, however when it breaks the wire and goes to spool up around the "forks" in the rethread block it spins maybe 1-2 times then stops and comes up with the re-thread failure.

Have replaced the re-thread module and the re-thread cable.
Any ideas?
