Does anyone know why a countersink would be punching harder than it should?

It's literally deforming the sheet metal. The bottom die is correct, and the height of the tool has been factored in.

The p-code haven't been messed with (to my knowledge) and the top position is set at .400 - I'm not sure why the countersink is coming out with rings like Saturn and smashing the sheet metal into deformed blobs.
 
I'm no press tool man so these are WAGs

1 the pre punched hole is too small

2 the metal thickness isn't what you think it is

Yes, the hole was .014 to large and the tip of the tool was broken.

Yes! # 2 - the thickness of the material was off. So I learned something new today.
