Mill head preventive maintenance interval

I have an Acer 3vk knee mill with a varispeed type head. While there is no rattly noises, it does operate louder than a similar, much newer Sharp machine I have at work, and I have no history on if/when the head has ever been gone through in its 25 years of use. I've read and watched some videos on replacing the plastic bushings and belts as part of preventive maintenance. Some sources say it's a repair task for when the head gets noisy (rattles), others say it's maintenance and if it gets noisy it's already doing damage to shafts/discs.

How often should these items be replaced? Should I run it til it starts getting "loose," then rebuild, or should I proactively do it now?
