Dividing Head Indexing Plate

I need some info on a plate I acquired recently to see if it's usable. Plate is 8.5" OD; 9/16" thick; ~1.1" center hole and drilled on both sides. Bolt or screw pattern is 4 hole and 2 hole.

Similar to K&T Model K - but the part number is either very much later or another maker

Actually I'm looking for the maker name. Since the center hole is smaller than the other plates I have and is 3 times thicker and drilled on both sides, I thought it was from a different maker.
