There are both right and left hand inserts in the photo.
Looks like it's for internal and or face threading.

Doesn't have to be a back-face. I run lots of left hand tools with M04. In my experience many if not most turning centers are more rigid when run M04.

Is there a number stamped on the holder?
That looks like a holder for "on edge" threading inserts that are held on with a special flat head screw. Like a TNMC-32V insert for an example.
