If you are machine tapping turn the last two or three threads in by hand. This negates the effect of holding the tap in a non concentric holder.

The question is; what is causing this condition? If the tap holder is not true to the centerline of the spindle the tap will cut over-size. A floating tap holder may correct the problem.

Are you reaming the hole before tapping, they make pipe reamers for all the sizes.

Thread mill it! I had to put 1000s of 1/8 27 holes in some gummy as hell aluminum extrusions, 36 in each, when using a tap it was hard to not scrap parts. One bad hole and the part was junk. Thread milling solved all the problems and did not create any new ones. I ran the rest of the job with no bad parts.
