What part of a bike is if from? What make of bike, or make of device?
is the barrel OD diameter important? Can you drill/ream the hole it goes into?

Go with a hardened steel M6 (8mm shaft diameter) shoulder screw and regular nut. Use a shoulder length long enough for free motion when the nut is bottomed, and die spacer washers if needed to control excess length.
Just ream the pivot points a little bit (may already be worn egg-shaped anyway), you can wrap the shoulder with a cut thin shim of brass or a tough polymer sheet to make up lost diameter. Keep the screw assembly lubed with a washout resistant grease for corrosion protection (and free motion, of course). Carry spares and the wrenches needed to replace if the thread undercut breaks on you.
