This is from my Sheldon T56 P lathe. There were 2 short setscrews in the threaded holes only 4 or 5 threads long which were folded over sideways and the nut pulled almost 1/2 way out of it's hole.

My first thought would be new holes with longer fine thread setscrews, but I'm seeking advice from the more experienced.

That is how some mfg. anchored in the nuts. If you want to re-install check for burrs, then file so the nut will slide in aligning the holes, Then tap. then use a bottom tap. Then blue Loc-Tite the screws If it was removed, the former owner felt he needed a new one. If you are replacing it, drill and tap new holes in another location

Edit: After looking at picture again, the treads are worn as they are suppose to be square (Acme) and the quill looks buggered up. If you want to by a new nut and screw, order from Green Bay Mfg. You will have to do some machining to both to make them fit. I am not sure you can buy new parts from Sheldon.
