I've been fighting with my machine all day to turn a simple arc. It simulates fine in process test but when it runs it does something completely different.

Looks like you’re profiling the back part of the radius with T060606 and G41 which is fine.
Then you profile the front half with T150615 and G42. How do those offsets compare?

It’s hard to tell from your sectioned part what we’re looking for.
What tool tip are you using in 06 and 15?

I'm using tip position 8 for both. 06 is the "leading edge" of the insert and 15 is the "trailing edge". So the difference between the two is 3/16 which is the insert width.
 
With tool tip 8 your Z offset should be the center of the radius.
Both 06 and 15 Z offsets should be the same.
 
I also noticed there is no Z move in N114 or N119 when you turn on comp. The Z comp may be being applied during the arc move which, instead of cutting a U, would cut a rounded V.

That’s kinda what pic 2 and 3 look like.
