    I worked as a tool trouble engineer with one of my duties was to study cutting tools, change their design to improve performance, and test different vendor products for best cost efficiency. One factor that could not be employed because of being time-consuming was a visual inspection. Yes, cutting tools from even from the best manufactures would occasionally include a bad tool. The quick trick is to look at a cutting tool's cutting edge with a loop to see that the edge has no obvious errors. one in a hundred, or one in a thousand error cutter might be the one that causes a premature failure or a wreck.
    With having skilled machine operators it would be logical to instruct them to loop-check cutting edges.
