Looking as in a replacement or a photo so you know what the missing numbers are.... I assume by the "face plate" you mean the threading plate?

You could get one new from Monarch for about $250 as of about 3-4 years ago. Or you can have one made for about $80:

I think the made plate is better thanks to a design improvement suggested by Cal Haines. If you just want a photo/design I have it filed away somewhere and could dig it up.
